NRS-11 Scale for Communicative Adults and Adolescents

| 4 | Moderate pain, no desire to pursue normal activities. |

| 6 | Severe pain, can still manage to focus on other things. |

| 8 | Extreme pain, cannot focus on anything else. |

| 10 | Pain as bad as it could be. |
FLACC Scale for Ages 0-7 and Noncommunicative Patients
